Recently, a collaborative effort between the Obstetrics Department, Intensive Care Unit, and Anesthesiology Department of the Second Hospital of Shandong University successfully treated a critically ill pregnant woman with a strong suspicion of amniotic fluid embolism at Jiyang Traditional Chinese Medicine Hospital. Once the patient's vital signs stabilized, the consulting physicians escorted her to the Intensive Care Unit at the Second Hospital of Shandong University for further treatment. Currently, the patient is recovering well and has been transferred to the Obstetrics ward.
On the morning of November 26th, Dr. Hong Fanzhen, Director of Obstetrics at the Second Hospital of Shandong University, received a call from Jiyang Family Planning Center regarding a pregnant woman experiencing continuous blood pressure drop during a cesarean section, posing a life-threatening situation. Dr. Hong, along with Dr. Zhao Hongyan from the Intensive Care Unit and Dr. Ma Jinfeng from Anesthesiology, immediately proceeded for consultation. Meanwhile, Director of Medical Affairs, Dr. Wang Ruoyi, received a supervisory call from the Women and Children's Division of the Jinan Municipal Health Commission, swiftly coordinating with the 120 emergency ambulance to reach Jiyang.
While en route to Jiyang, the consulting physicians urgently contacted local doctors and gathered detailed information indicating a suspected amniotic fluid embolism. The patient exhibited severe allergic shock symptoms with prominent cyanosis, sudden convulsions, and loss of consciousness. She had undergone endotracheal intubation for assisted breathing. Due to severe acidosis, her blood pressure was unmeasurable, indicating an extremely critical condition. The three doctors immediately provided remote guidance for treatment, including anti-allergic therapy, correction of acidosis, and increasing the concentration of vasopressor drugs as rescue measures. Simultaneously, to preempt potential coagulation dysfunction, they advised prompt preparation of blood products. Through proactive treatment, the patient's blood pressure gradually returned to normal, and the swift supplementation of blood products timely halted the progression of coagulation abnormalities.
Once the patient's condition stabilized, she was safely transported to the Intensive Care Unit at the Second Hospital of Shandong University around 5 PM, where she continued to receive anti-shock and anti-infection treatments to minimize cardiovascular, cerebrovascular, and pulmonary complications. On the morning of November 27th, the patient regained consciousness, and her vital signs remained stable. On November 30th, she was transferred to the Obstetrics Rehabilitation unit.
The Second Hospital of Shandong University serves as the critical obstetric and gynecological rescue center for Shandong Province and Jinan City, taking on the responsibility of treating critically ill pregnant women from the surrounding areas. In recent years, under the hospital's coordinated arrangements, in addition to actively establishing a green channel for the rapid admission of critically ill pregnant women, multidisciplinary teams, including Obstetrics, Intensive Care, Anesthesiology, Neonatology, and others, have been dispatched multiple times to local hospitals to treat critically ill pregnant women. This has earned the trust of numerous grassroots hospitals and received high praise from leaders at various levels of health commissions, making a positive contribution to ensuring the safety of pregnant women.
